• a. Photocopy of Form 138 or the report card.
  b. For Filipino citizen: Photocopy of PSA / NSO Registered Birth
  Certificate (Bring original copy for verification)
  c. For Non-Filipino citizen: Photocopies of Passport and I-card
  (Bring original copy for verification).

2. Accept an interview from Admissions Office.
3. Pay the non-refundable entrance examination fee.
4. Obtain test schedule for an entrance examination from the
Admissions’ Office.
5. Undergo examination at the assigned venue on the
scheduled date.
6. Proceed to Director’s Office for a final interview.

For Successful Applicants:

• a. Secure permit to enroll or Assessment slip from the
  Office of Admissions.
  b. Pay the non-refundable reservation fee, which is deductible
  from the tuition fee upon enrollment.
